As undifferentiated cells, stem cells have the power to develop into specialized cells that can be used to repair and replace damaged tissues.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-47\"><span style=\"font-weight: 400;\">It is this ability to specialize that makes stem cells such an exciting focus for research, effectively broadening the horizon of what was thought possible in medicine.\u00a0\u00a0<\/span><\/p>\n<p class=\"ai-optimize-48\"><span style=\"font-weight: 400;\">Today stem cells are being used to treat a wide range of conditions, from blood cancers like Leukemia to metabolic disorders like Krabbe Disease. However, clinical trials in recent years have shown that stem cells may also have applications in treating as-of-yet incurable eye conditions, specifically retinal diseases.\u00a0\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3 class=\"ai-optimize-49\"><span style=\"font-weight: 400;\">Age-Related Macular Degeneration<\/span><\/h3>\n<p class=\"ai-optimize-50\"><span style=\"font-weight: 400;\">Age-related macular degeneration (AMD) is a common retinal disease that arises as a result of damage to the macular through ageing. A small part of the retina &#8211; the light sensitive tissue that sits at the back of the eye &#8211; the macular is responsible for the sharp, central area of one\u2019s vision.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-51\"><span style=\"font-weight: 400;\">By causing the macular to degenerate, AMD makes activities like driving, reading and watching TV incredibly hard. It also has the unnerving effect of making faces difficult to recognize. [<a href=\"https:\/\/www.nhs.uk\/conditions\/age-related-macular-degeneration-amd\/symptoms\/\" target=\"_blank\" rel=\"noopener\">1<\/a>]<\/span><\/p>\n<p class=\"ai-optimize-52\"><span style=\"font-weight: 400;\">Unfortunately there is so far no cure for AMD. Available treatments currently range from the prescription of exercise and dietary supplements to eye-injections and laser treatment, all of which merely slow down the progression of AMD.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-53\"><span style=\"font-weight: 400;\">However, new research suggests that the use of stem cells may go beyond just slowing down the progression of AMD. Incredibly, it may actually be able to reverse the damage caused to the macular. [<a href=\"https:\/\/www.nei.nih.gov\/learn-about-eye-health\/eye-conditions-and-diseases\/age-related-macular-degeneration\" target=\"_blank\" rel=\"noopener\">2<\/a>]<\/span><\/p>\n<p class=\"ai-optimize-54\"><span style=\"font-weight: 400;\">In a study published in January of this year, researchers at Duke-NUS Medical School looked at using a human retina-specific protein, LN523, for culturing human embryonic stem cells (ESCs). ESCs are pluripotent, meaning they can both self-renew and differentiate into specialized cell types. The researchers\u2019 aim was to create a retinal environment ideal for the differentiation of these pluripotent stem cells into photoreceptor cells. These photoreceptor cells are the ones responsible for absorbing light and converting it into signals that trigger biological processes (seeing, in other words).\u00a0<\/span><\/p>\n<p class=\"ai-optimize-55\"><span style=\"font-weight: 400;\">Miraculously, results showed that by introducing LN523, researchers were able to cause the stem cells to differentiate into photoreceptor progenitors within just 32 days. Even more surprising was that these new photoreceptor cells successfully engrafted themselves in both rodent and rabbit retinas. Afterwards, in assessing the visual behaviour of the rodents who underwent cell-transplantation, researchers also found significant improvements in vision. <\/span><\/p>\n<p class=\"ai-optimize-56\"><span style=\"font-weight: 400;\">Hence, these promising findings suggest that stem cells may offer the means for potentially replacing lost photoreceptor cells in degenerative retinal conditions, like AMD, thereby offering hope for a possible cure. [<a href=\"https:\/\/www.cell.com\/molecular-therapy-family\/molecular-therapy\/fulltext\/S1525-0016(22)00716-X#supplementaryMaterial\" target=\"_blank\" rel=\"noopener\">3<\/a>] [<a href=\"https:\/\/www.forbes.com\/sites\/williamhaseltine\/2023\/07\/29\/stem-cell-therapies-for-retinal-disorders-are-a-sight-for-sore-eyes\/?sh=fb3cc816c4d5\" target=\"_blank\" rel=\"noopener\">4<\/a>]\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3 class=\"ai-optimize-57\"><span style=\"font-weight: 400;\">Stargardt&#8217;s Macular Dystrophy<\/span><\/h3>\n<p class=\"ai-optimize-58\"><span style=\"font-weight: 400;\">Stargardt&#8217;s Macular Dystrophy (SMD), or Stargardt\u2019s disease, like AMD, is a condition affecting the macula. Unlike AMD, SMD is a rare inherited condition. Passed down through an<\/span><span style=\"font-weight: 400;\"> autosomal recessive pattern, children of parents with changed genes have a 25% chance of inheriting the mutated ABCA4 gene.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-59\"><span style=\"font-weight: 400;\">This mutation affects the coding for the protein responsible for transporting wastes from the photoreceptor cells in the retina. When these toxic waste molecules pile up in the photoreceptor cells it leads to them deteriorating, along with the vision of the affected person.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-60\"><span style=\"font-weight: 400;\">SMD, like AMD, causes degeneration in the central area of vision. Additionally, however, SMD can also result in other vision issues, such as sensitivity to light, the appearance of blank or black spots, and a reduction in colour perception.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-61\"><span style=\"font-weight: 400;\">The onset of SMD usually occurs in adolescence, with related visual impairment becoming pronounced by early adulthood. Currently there is no cure for SMD, with the only available option for sufferers of the condition being the protection of any remaining sight through due-diligence, dietary supplements and lifestyle changes.\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0<\/span><\/p>\n<p class=\"ai-optimize-62\"><span style=\"font-weight: 400;\">Similarly to the way that pluripotent embryo-derived stem cells (ESCs) are being explored for the treatment of AMD, clinical trials are also underway for the treatment of SMD with pluripotent stem cells. These trials seek to repair and replace damaged retinal cells with stem cell-derived healthy cells.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-63\"><span style=\"font-weight: 400;\">Accordingly, in one such trial from 2015, researchers investigated the \u2018safety and tolerability\u2019 <\/span><span style=\"font-weight: 400;\">of a sub-retinal transplantation of ESCs which had differentiated into retinal pigment epithelium &#8211; the cells beneath the photoreceptors in the retina. [<a href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(14)61376-3\/fulltext#articleInformation\" target=\"_blank\" rel=\"noopener\">5<\/a>]<\/span><\/p>\n<p class=\"ai-optimize-64\"><span style=\"font-weight: 400;\">The trial was conducted with nine patients aged between 18 and 55, and nine patients aged 55 or over, all of whom had SMD.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-65\"><span style=\"font-weight: 400;\">Amazingly, vision-related quality-of-life measures increased by between 8-20 points 3 to 12 months after transplantation. This trial, whilst conducted with a relatively small pool, suggests that stem cell therapy is not only a safe option for the treatment of SMD, but a viable one, too.\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3 class=\"ai-optimize-66\"><span style=\"font-weight: 400;\">Retinitis Pigmentosa\u00a0<\/span><\/h3>\n<p class=\"ai-optimize-67\"><span style=\"font-weight: 400;\">Retinitis Pigmentosa (RP) is a commonly inherited retinal disease which causes the progressive loss of photoreceptor cells, resulting in the gradual decline in visual acuity. Like SMD, the onset of RP may manifest in childhood, but the first symptoms are usually spotted in adolescence and early adulthood.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-68\"><span style=\"font-weight: 400;\">These symptoms normally progress from nyctalopia, otherwise known as night-blindness, to the deterioration of peripheral vision, to the eventual loss of the central area of vision owing to the degeneration of the macula.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-69\"><span style=\"font-weight: 400;\">Like AMD and SMD, RP currently remains incurable. However, recent clinical trials have suggested that by transplanting specialized stem cells, either ESCs or induced pluripotent stem cells (iPSCs) such as those derived from adult human tissues, into the retina, photoreceptors can be both repaired and replaced. [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3939754\/\" target=\"_blank\" rel=\"noopener\">6<\/a>]<\/span><\/p>\n<p class=\"ai-optimize-70\"><span style=\"font-weight: 400;\">As with AMD and SMD, stem cell treatment routes for RP are still being explored and have not yet moved beyond the clinical trial stage. However, there is a significant amount of research and evidence to indicate the viability and potential of such treatments in the near future.<\/span><\/p>\n<p But when feeling down persists for weeks, or even months, it can be a sign of a mental health condition, like depression.<\/p><p>Although today people are well versed and more understanding of depression, some sufferers still find it difficult to find treatments.<\/p><h3>What is depression?<\/h3><p>Depression is a common mental health condition. It is characterised by low mood, feelings of hopelessness, losing interest in things which previously brought a sense of enjoyment, and feeling tearful.<\/p><p>Depression can also manifest in physical ways. Sleeping poorly, feeling tired, experiencing aches, pains, a diminished appetite, and decreased libido can all be impacts of depression. The symptoms range from mild to severe. In mild cases, a persistent low mood may be the cause, while severe cases may result in suicidal feelings or a desire to self harm <a href=\"https:\/\/www.nhs.uk\/mental-health\/conditions\/depression-in-adults\/overview\/\">[1]<\/a>.<\/p><p>Sometimes the causes of depression can be obvious, such as a bereavement or a traumatic event. Other times there might be no known cause, although family history could be the reason. Whatever the reason, once a person has identified they may be depressed, they should go and see their GP.<\/p><h3>What treatments are available?<\/h3><p>There are a plethora of treatments for depression that a doctor can prescribe, from talking therapies to pharmaceutical medications to exercise and more. Sometimes the GP may recommend one treatment or a combination of treatments but they will help support patients and signpost them to services which can bolster their support network.<\/p><p>The good news is that depression is among the most treatable mental health disorders.\u00a0 Between 80% and 90% percent of people with depression eventually respond well to treatment. Almost all patients gain some relief from their symptoms <a href=\"https:\/\/www.psychiatry.org\/patients-families\/depression\/what-is-depression#:~:text=Depression%20is%20among%20the%20most,some%20relief%20from%20their%20symptoms.\">[2]<\/a>.<\/p><h3>Treatment Resistant Depression<\/h3><p>For those with major depressive disorder, however, treatment can be harder to find. With up to one third of adults with the condition failing to respond to treatment, new avenues and treatments are being sought.<\/p><p>\u201cAlthough there is some disagreement as to how to define treatment-resistant depression, a patient is generally considered to have it if the individual hasn\u2019t responded to adequate doses of two different antidepressants taken for a sufficient duration of time, which is usually six weeks,\u201d explains Jaskaran Singh, M.D., Senior Director of Neuroscience, Janssen Pharmaceuticals <a href=\"https:\/\/www.jnj.com\/health-and-wellness\/4-facts-about-treatment-resistant-depression\">[3]<\/a>.<\/p><p>The theory that depression may have causes currently unknown by scientists is one theory researchers are exploring to try and combat resistance to treatment.<\/p><p>\u201cOne of the more modern theories is that depression creates inflammation in the brain, or that inflammation in the brain creates depression. Traditional antidepressants only affect neurotransmitters, so this may be why some patients don\u2019t respond to them.\u201d\u00a0 says Alexander Papp, M.D., a psychiatrist at UC San Diego Health <a href=\"https:\/\/www.jnj.com\/health-and-wellness\/4-facts-about-treatment-resistant-depression\">[4]<\/a>.<\/p><p>Whether or not this is the case, researchers are still trying to find solutions to Treatment Resistant Depression.<\/p><h3>The Potential of Cord Blood Therapy<\/h3><p>One study using umbilical cord blood cells delivered promising results. The study administered concentrated umbilical cord blood cells at a dose of 250 million cells via weekly injections over 4 weeks with 1 week intervals, to women who suffered with Treatment Resistant Depression.\u00a0 The results were tested against a control group who received a placebo.<\/p><p>The study found that the therapeutic potential of umbilical cord blood cell concentrate can be used to overcome treatment resistance formed in depressive patients <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7087609\/\">[5]<\/a>. Other studies have also found that cellular therapies may have a role in combating Treatment Resistant Depression <a href=\"https:\/\/www.mdpi.com\/1999-4923\/15\/3\/814\">[6]<\/a> <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC8474995\/\">[7]<\/a>.<\/p><p>These studies give hope for finding new treatments.
