It\u2019s also the first time intravenous (IV) delivery will be trialled for the delivery of cell therapy for heart failure. <\/span><a href=\"https:\/\/louisville.edu\/medicine\/news\/uofl-cardiologist-leading-clinical-trial-for-high-potential-new-therapy-for-heart-failure\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[1]<\/span><\/a><\/p>\n<p class=\"ai-optimize-152\"><span style=\"font-weight: 400;\">It is hoped that this pioneering approach could change the way that heart failure patients are treated.\u00a0\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3 class=\"ai-optimize-153\"><span style=\"font-weight: 400;\">What is heart failure?<\/span><\/h3>\n<p class=\"ai-optimize-154\"><span style=\"font-weight: 400;\">Heart failure arises when the heart cannot pump blood around the body properly. This results in the body being unable to receive the oxygen it needs to function normally.<\/span><\/p>\n<p class=\"ai-optimize-155\"><span style=\"font-weight: 400;\">Heart failure can arise from various causes, with some of the most common being:<\/span><\/p>\n<ul>\n<li class=\"ai-optimize-156\" style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Cardiomyopathy: A condition where the heart&#8217;s muscular wall thickens, making it difficult for the heart to effectively pump blood throughout the body.<\/span><\/li>\n<li class=\"ai-optimize-157\" style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Heart Attack: A heart attack can cause lasting damage to the heart, impairing its ability to circulate blood efficiently.<\/span><\/li>\n<li class=\"ai-optimize-158\" style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">High Blood Pressure: Chronic high blood pressure puts strain on the heart, gradually reducing its pumping efficiency.<\/span><\/li>\n<li class=\"ai-optimize-159\" style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Abnormal Heart Rhythms: An irregular, too slow, or too fast heartbeat can disrupt the heart\u2019s normal pumping function. <\/span><a href=\"https:\/\/www.bhf.org.uk\/informationsupport\/conditions\/heart-failure?gad_source=1&amp;gclid=Cj0KCQjww5u2BhDeARIsALBuLnP2GAM0j5TBp0pX-OL9l3Vgzt8yZWBcKKk8j5T0m_JI8pHKPqZKiY8aAi02EALw_wcB&amp;gclsrc=aw.ds\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[2]<\/span><\/a><\/li>\n<\/ul>\n<p class=\"ai-optimize-160\"><span style=\"font-weight: 400;\">It is estimated that heart failure affects at least 64 million people worldwide. <\/span><a href=\"https:\/\/doi.org\/10.1093\/eurjpc\/zwaa147\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[3]<\/span><\/a><\/p>\n<p class=\"ai-optimize-161\"><span style=\"font-weight: 400;\">Heart failure usually gets worse over time. However, while there is no cure, there are steps that can be taken to treat and manage symptoms, including surgery, having a pacemaker implanted, medication, and lifestyle changes. <\/span><a href=\"https:\/\/www.nhs.uk\/conditions\/heart-failure\/\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[4]<\/span><\/a><\/p>\n<p>&nbsp;<\/p>\n<h3 class=\"ai-optimize-162\"><span style=\"font-weight: 400;\">What is the new stem cell trial for heart disease?<\/span><\/h3>\n<p class=\"ai-optimize-163\"><span style=\"font-weight: 400;\">The trial, which has been dubbed \u2018CATO\u2019, will enroll 60 participants across 3 locations, all of whom would have suffered from ischemic cardiomyopathy &#8211; heart failure arising from the damage caused by heart attack.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-164\"><span style=\"font-weight: 400;\">Conducted on a randomized, double blind and placebo controlled basis to ensure rigorous scientific standards, participants will be split into three groups: a control group, a single dose group and a multiple dose group.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-165\"><span style=\"font-weight: 400;\">All participants will receive four infusions delivered via IV over the course of 2 months.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-166\"><span style=\"font-weight: 400;\">The control group will receive four doses of placebo, the single dose group will receive one dose of umbilical cord-derived mesenchymal stem cells (UC-MSCs) and three doses of placebo, and the multiple dose group will receive four doses of UC-MSCs.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-167\"><span style=\"font-weight: 400;\">The first trial to test the delivery of cell therapy for heart failure in multiple doses, CATO will not only investigate the effectiveness of using UC-MSCs for treating heart failure, but also the effectiveness of their delivery in multiple infusions.<\/span><\/p>\n<p class=\"ai-optimize-168\"><span style=\"font-weight: 400;\">Participants will be monitored at intervals of 2 hours, 1 week, and 2 months after each infusion and then followed for 6 months after all four infusions to monitor the safety and efficacy of the therapy. <\/span><a href=\"https:\/\/clinicaltrials.gov\/study\/NCT06145035\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[5]<\/span><\/a><\/p>\n<p>&nbsp;<\/p>\n<h3 class=\"ai-optimize-169\"><span style=\"font-weight: 400;\">Why are umbilical cord stem cells being used in the heart failure trial?<\/span><\/h3>\n<p class=\"ai-optimize-170\"><span style=\"font-weight: 400;\">Previous studies have shown that mesenchymal stem cells derived from the umbilical cord have abilities that make them ideal in the treatment of cardiovascular diseases.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-171\"><span style=\"font-weight: 400;\">These include the ability to differentiate into cardiovascular progenitor cells, to modulate immune responses, and promote growth factors. <\/span><a href=\"https:\/\/doi.org\/10.7759\/cureus.49645\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[6]<\/span><\/a> <a href=\"https:\/\/doi.org\/10.1161\/CIRCRESAHA.117.310712\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[7]<\/span><\/a><\/p>\n<p class=\"ai-optimize-172\"><span style=\"font-weight: 400;\">These cells, harvested from donated umbilical cords, have shown promise in treating a range of conditions, including ulcerative colitis, Crohn\u2019s disease, and even COVID-19. <\/span><a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/10.1111\/1440-1681.12321\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[8]<\/span><\/a> <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5753687\/\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[9]<\/span><\/a> <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC10573267\/\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[10]<\/span><\/a><\/p>\n<p class=\"ai-optimize-173\"><span style=\"font-weight: 400;\">Now, for the first time in the United States, they are being tested in heart failure patients.<\/span><\/p>\n<p class=\"ai-optimize-174\"><span style=\"font-weight: 400;\">UC-MSCs offer several advantages in this area over other types of stem cells.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-175\"><span style=\"font-weight: 400;\">Most notably, UC-MSCs can be isolated, stored frozen and then expanded into large quantities, making them readily available &#8220;off the shelf&#8221; But when feeling down persists for weeks, or even months, it can be a sign of a mental health condition, like depression.<\/p><p>Although today people are well versed and more understanding of depression, some sufferers still find it difficult to find treatments.<\/p><h3>What is depression?<\/h3><p>Depression is a common mental health condition. It is characterised by low mood, feelings of hopelessness, losing interest in things which previously brought a sense of enjoyment, and feeling tearful.<\/p><p>Depression can also manifest in physical ways. Sleeping poorly, feeling tired, experiencing aches, pains, a diminished appetite, and decreased libido can all be impacts of depression. The symptoms range from mild to severe. In mild cases, a persistent low mood may be the cause, while severe cases may result in suicidal feelings or a desire to self harm <a href=\"https:\/\/www.nhs.uk\/mental-health\/conditions\/depression-in-adults\/overview\/\">[1]<\/a>.<\/p><p>Sometimes the causes of depression can be obvious, such as a bereavement or a traumatic event. Other times there might be no known cause, although family history could be the reason. Whatever the reason, once a person has identified they may be depressed, they should go and see their GP.<\/p><h3>What treatments are available?<\/h3><p>There are a plethora of treatments for depression that a doctor can prescribe, from talking therapies to pharmaceutical medications to exercise and more. Sometimes the GP may recommend one treatment or a combination of treatments but they will help support patients and signpost them to services which can bolster their support network.<\/p><p>The good news is that depression is among the most treatable mental health disorders.\u00a0 Between 80% and 90% percent of people with depression eventually respond well to treatment. Almost all patients gain some relief from their symptoms <a href=\"https:\/\/www.psychiatry.org\/patients-families\/depression\/what-is-depression#:~:text=Depression%20is%20among%20the%20most,some%20relief%20from%20their%20symptoms.\">[2]<\/a>.<\/p><h3>Treatment Resistant Depression<\/h3><p>For those with major depressive disorder, however, treatment can be harder to find. With up to one third of adults with the condition failing to respond to treatment, new avenues and treatments are being sought.<\/p><p>\u201cAlthough there is some disagreement as to how to define treatment-resistant depression, a patient is generally considered to have it if the individual hasn\u2019t responded to adequate doses of two different antidepressants taken for a sufficient duration of time, which is usually six weeks,\u201d explains Jaskaran Singh, M.D., Senior Director of Neuroscience, Janssen Pharmaceuticals <a href=\"https:\/\/www.jnj.com\/health-and-wellness\/4-facts-about-treatment-resistant-depression\">[3]<\/a>.<\/p><p>The theory that depression may have causes currently unknown by scientists is one theory researchers are exploring to try and combat resistance to treatment.<\/p><p>\u201cOne of the more modern theories is that depression creates inflammation in the brain, or that inflammation in the brain creates depression. Traditional antidepressants only affect neurotransmitters, so this may be why some patients don\u2019t respond to them.\u201d\u00a0 says Alexander Papp, M.D., a psychiatrist at UC San Diego Health <a href=\"https:\/\/www.jnj.com\/health-and-wellness\/4-facts-about-treatment-resistant-depression\">[4]<\/a>.<\/p><p>Whether or not this is the case, researchers are still trying to find solutions to Treatment Resistant Depression.<\/p><h3>The Potential of Cord Blood Therapy<\/h3><p>One study using umbilical cord blood cells delivered promising results. The study administered concentrated umbilical cord blood cells at a dose of 250 million cells via weekly injections over 4 weeks with 1 week intervals, to women who suffered with Treatment Resistant Depression.\u00a0 The results were tested against a control group who received a placebo.<\/p><p>The study found that the therapeutic potential of umbilical cord blood cell concentrate can be used to overcome treatment resistance formed in depressive patients <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7087609\/\">[5]<\/a>. Other studies have also found that cellular therapies may have a role in combating Treatment Resistant Depression <a href=\"https:\/\/www.mdpi.com\/1999-4923\/15\/3\/814\">[6]<\/a> <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC8474995\/\">[7]<\/a>.<\/p><p>These studies give hope for finding new treatments.
