It is often fatal if untreated.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-94\"><span style=\"font-weight: 400;\">Approximately 85% of Krabbe disease cases are the infantile subtypes. <\/span><a href=\"https:\/\/doi.org\/10.1182\/blood.2020005477\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[1]<\/span><\/a><\/p>\n<p>&nbsp;<\/p>\n<h3 class=\"ai-optimize-95\"><span style=\"font-weight: 400;\">What are the current treatment options for Krabbe disease?<\/span><\/h3>\n<p class=\"ai-optimize-96\"><span style=\"font-weight: 400;\">Currently, the only effective treatment option for Krabbe disease is a transplant of haematopoietic stem cells (HSCs), which are found in bone marrow, peripheral blood, and umbilical cord blood. <\/span><a href=\"https:\/\/doi.org\/10.1182\/blood.2020005477\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[2]<\/span><\/a><\/p>\n<p class=\"ai-optimize-97\"><span style=\"font-weight: 400;\">By transplanting HSCs from a donor to a Krabbe disease patient, the patient\u2019s unhealthy cells lacking in the GALC enzyme are eventually replaced by healthy red blood cells, white blood cells and platelets derived from HSCs.<\/span><\/p>\n<p class=\"ai-optimize-98\"><span style=\"font-weight: 400;\">These healthy blood cells can then work to populate the brain with GALC enzyme activity, reducing the breakdown of myelin and thereby stabilise cognitive function. <\/span><a href=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/6039-krabbe-disease-globoid-cell-leukodystrophy\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[3]<\/span><\/a><\/p>\n<p class=\"ai-optimize-99\"><span style=\"font-weight: 400;\">Stem cell transplants using haematopoietic stem cells from cord blood, specifically, have been shown to be highly effective in improving neurological outcomes if the transplant is performed before the development of symptoms. <\/span><a href=\"https:\/\/doi.org\/10.1212\/WNL.0000000000004418\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[4]<\/span><\/a><\/p>\n<p class=\"ai-optimize-100\"><span style=\"font-weight: 400;\">While HSCT remains the only viable treatment option for Krabbe disease, it is not a cure.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-101\"><span style=\"font-weight: 400;\">It also does not combat the peripheral neuropathy occasioned by Krabbe disease, a condition affecting the nerves beyond the brain and spinal cord, leading to decline in motor function. <\/span><a href=\"https:\/\/doi.org\/10.1186\/s13023-019-1018-4\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[5]<\/span><\/a><span style=\"font-weight: 400;\">\u00a0\u00a0\u00a0\u00a0\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3 class=\"ai-optimize-102\"><span style=\"font-weight: 400;\">What is the new gene therapy and how does it improve stem cell transplant outcomes?<\/span><\/h3>\n<p class=\"ai-optimize-103\"><span style=\"font-weight: 400;\">Developed by Forge Biologics, the FBX-101 therapy works by delivering a copy of the GALC gene to cells in the nervous system, which improves myelination (the process by which the myelin sheath forms) and, crucially, motor function.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-104\"><span style=\"font-weight: 400;\">It is designed to be administered intravenously after the current standard of care, a haematopoietic stem cell transplant.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-105\"><span style=\"font-weight: 400;\">In its early phase trial, REKLAIM, FBX-101 was shown to improve motor function in all five of the patients who underwent treatment.<\/span><\/p>\n<p class=\"ai-optimize-106\"><span style=\"font-weight: 400;\">Building on these promising results, the Innovation Passport designation means that FBX-101 will be able to enter the Innovative Licensing and Access Pathway (ILAP), which accelerates both market and regulatory access in the UK. <\/span><a href=\"https:\/\/www.forgebiologics.com\/forge-biologics-novel-aav-gene-therapy-fbx-101-for-patients-with-krabbe-disease-is-granted-uks-innovation-passport-designation\/\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[6]\u00a0 <\/span><\/a>In the United States, FBX-101 has also received FDA Fast Track designation, alongside Orphan Drug and Rare Pediatric Disease designations, underscoring its potential to speed development and review for patients with Krabbe disease. <a infantile form, and early intervention is the key to preventing severe neurological damage.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-109\"><span style=\"font-weight: 400;\">Ideally, the patient would receive a transplant within 30 days in order to have the best chance of improved neurological and transplant outcomes. <\/span><a href=\"https:\/\/doi.org\/10.1182\/bloodadvances.2021006094\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[8]<\/span><\/a><\/p>\n<p class=\"ai-optimize-110\"><span style=\"font-weight: 400;\">Symptoms of Krabbe disease often appear within the first few months of life, and once they start, the deterioration of the nervous system is fast and irreversible.\u00a0<\/span><\/p>\n<p class=\"ai-optimize-111\"><span style=\"font-weight: 400;\">By the time symptoms are noticeable, significant damage has already occurred, limiting the effectiveness of available treatments.<\/span><\/p>\n<p class=\"ai-optimize-112\"><span style=\"font-weight: 400;\">Newborn screening allows for early diagnosis before symptoms develop, enabling early intervention through HSCT, which is at its most But when feeling down persists for weeks, or even months, it can be a sign of a mental health condition, like depression.<\/p><p>Although today people are well versed and more understanding of depression, some sufferers still find it difficult to find treatments.<\/p><h3>What is depression?<\/h3><p>Depression is a common mental health condition. It is characterised by low mood, feelings of hopelessness, losing interest in things which previously brought a sense of enjoyment, and feeling tearful.<\/p><p>Depression can also manifest in physical ways. Sleeping poorly, feeling tired, experiencing aches, pains, a diminished appetite, and decreased libido can all be impacts of depression. The symptoms range from mild to severe. In mild cases, a persistent low mood may be the cause, while severe cases may result in suicidal feelings or a desire to self harm <a href=\"https:\/\/www.nhs.uk\/mental-health\/conditions\/depression-in-adults\/overview\/\">[1]<\/a>.<\/p><p>Sometimes the causes of depression can be obvious, such as a bereavement or a traumatic event. Other times there might be no known cause, although family history could be the reason. Whatever the reason, once a person has identified they may be depressed, they should go and see their GP.<\/p><h3>What treatments are available?<\/h3><p>There are a plethora of treatments for depression that a doctor can prescribe, from talking therapies to pharmaceutical medications to exercise and more. Sometimes the GP may recommend one treatment or a combination of treatments but they will help support patients and signpost them to services which can bolster their support network.<\/p><p>The good news is that depression is among the most treatable mental health disorders.\u00a0 Between 80% and 90% percent of people with depression eventually respond well to treatment. Almost all patients gain some relief from their symptoms <a href=\"https:\/\/www.psychiatry.org\/patients-families\/depression\/what-is-depression#:~:text=Depression%20is%20among%20the%20most,some%20relief%20from%20their%20symptoms.\">[2]<\/a>.<\/p><h3>Treatment Resistant Depression<\/h3><p>For those with major depressive disorder, however, treatment can be harder to find. With up to one third of adults with the condition failing to respond to treatment, new avenues and treatments are being sought.<\/p><p>\u201cAlthough there is some disagreement as to how to define treatment-resistant depression, a patient is generally considered to have it if the individual hasn\u2019t responded to adequate doses of two different antidepressants taken for a sufficient duration of time, which is usually six weeks,\u201d explains Jaskaran Singh, M.D., Senior Director of Neuroscience, Janssen Pharmaceuticals <a href=\"https:\/\/www.jnj.com\/health-and-wellness\/4-facts-about-treatment-resistant-depression\">[3]<\/a>.<\/p><p>The theory that depression may have causes currently unknown by scientists is one theory researchers are exploring to try and combat resistance to treatment.<\/p><p>\u201cOne of the more modern theories is that depression creates inflammation in the brain, or that inflammation in the brain creates depression. Traditional antidepressants only affect neurotransmitters, so this may be why some patients don\u2019t respond to them.\u201d\u00a0 says Alexander Papp, M.D., a psychiatrist at UC San Diego Health <a href=\"https:\/\/www.jnj.com\/health-and-wellness\/4-facts-about-treatment-resistant-depression\">[4]<\/a>.<\/p><p>Whether or not this is the case, researchers are still trying to find solutions to Treatment Resistant Depression.<\/p><h3>The Potential of Cord Blood Therapy<\/h3><p>One study using umbilical cord blood cells delivered promising results. The study administered concentrated umbilical cord blood cells at a dose of 250 million cells via weekly injections over 4 weeks with 1 week intervals, to women who suffered with Treatment Resistant Depression.\u00a0 The results were tested against a control group who received a placebo.<\/p><p>The study found that the therapeutic potential of umbilical cord blood cell concentrate can be used to overcome treatment resistance formed in depressive patients <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7087609\/\">[5]<\/a>. Other studies have also found that cellular therapies may have a role in combating Treatment Resistant Depression <a href=\"https:\/\/www.mdpi.com\/1999-4923\/15\/3\/814\">[6]<\/a> <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC8474995\/\">[7]<\/a>.<\/p><p>These studies give hope for finding new treatments.
