hover_enabled=&#8221;0&#8243; global_colors_info=&#8221;{}&#8221; sticky_enabled=&#8221;0&#8243;]\n<p><span style=\"font-weight: 400;\">Recent research indicates that umbilical cord exosomes have huge potential in the field of regenerative medicine.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In this blog we\u2019ll explore what exosomes are, where they\u2019re found, and how they could be used in developing new, life changing therapies for a range of conditions.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3><span style=\"font-weight: 400;\">What are exosomes?<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Exosomes are nano-sized particles that contain key biomolecules like proteins and lipids and travel between cells like messengers, regulating numerous biological processes including inflammatory responses.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Initially thought to be responsible for conveying waste away from cells, in recent years numerous trials and studies have explored their intercellular communicatory function and the\u00a0 potential this offers in a range of therapeutic applications.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3><span style=\"font-weight: 400;\">Where are exosomes found?<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Exosomes are produced by virtually every cell in the body, including stem cells.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Excitingly they can be derived from the mesenchymal stem cells that exist within both umbilical cord blood and umbilical cord tissue.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Due to the low immunogenicity (the likelihood of being attacked by the immune system when transplanted) of mesenchymal stem cells derived from the umbilical cord, along with their non-invasive harvest procedure and ease of expansion in vitro, umbilical cord MSC exosomes (UC-MSC-Exo) are well suited for research and potentially therapeutic purposes. <\/span><a href=\"https:\/\/stemcellres.biomedcentral.com\/articles\/10.1186\/s13287-020-01782-9\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[1]<\/span><\/a><\/p>\n<p>&nbsp;<\/p>\n<h3><span style=\"font-weight: 400;\">What can umbilical cord exosomes do?<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Below are a few examples of recent trials utilising UC-MSC-Exo in the treatment of various conditions.<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><b>Heal wounds and repair nerves<\/b><\/p>\n<p><span style=\"font-weight: 400;\">A 2022 study investigating the use of UC-MSC-Exo in treating cutaneous nerve damage and wound healing showed that exosomes have the ability to promote skin and nerve regeneration.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Studied in vitro, researchers found that UC-MSC-Exo promoted the migration and proliferation of skin fibroblasts &#8211; cells that play a key role in forming connective tissue.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Additionally, UC-MSC-Exo were also found to promote the secretion of nerve growth factors by fibroblasts, suggesting that exosomes may enhance wound healing by promoting nerve repair.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Tested in vivo in a mouse model, the study showed that the wounds of the treatment group who received UC-MSC-Exo healed faster than the wounds of the group who received a control medium.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The study concluded that UC-MSC-Exo produced therapeutic effects by promoting skin and nerve regeneration. <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC9279568\/\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[2]<\/span><\/a><\/p>\n<p>&nbsp;<\/p>\n<p><b>Slow the progress of osteoarthritis<\/b><\/p>\n<p><span style=\"font-weight: 400;\">Another study from 2022 demonstrated that exosomes derived from the umbilical cord have the potential to be used as a treatment method for osteoarthritis (OA).<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Using a rat model, researchers undertook a surgical procedure designed to recreate the effects of knee osteoarthritis. They then administered injections of either UC-MSC-Exo or saline solution. After 8 weeks, knee samples were taken to assess the progression of the disease.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Researchers discovered that in the knees of rats who had received injections of exosomes the progression of OA had been halted and severe damage to knee cartilage prevented.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">One of the key reasons behind this, the researchers found, was that the exosomes promoted the migration and proliferation of chondrocytes &#8211; cells that make up cartilage. They also found that the exosomes helped to inhibit the secretion of pro-inflammatory factors, as well as regulate immune responses; contributing factors in cartilage regeneration. <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC9577719\/\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[3]<\/span><\/a><\/p>\n<p>&nbsp;<\/p>\n<p><b>Alleviate liver damage and disease<\/b><\/p>\n<p><span style=\"font-weight: 400;\">A study from 2020 showed that exosomes derived from umbilical cord mesenchymal stem cells were able to secrete the molecule miR-455-3p which helped to suppress inflammatory immune responses and inhibit signalling pathways in order to improve liver damage. <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6979401\/\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[4]<\/span><\/a><\/p>\n<p><span style=\"font-weight: 400;\">Another study showed that exosomes derived from umbilical cord mesenchymal stem cells have antioxidant effects.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A contributing factor to the progression of liver damage, oxidative stress occurs when there is an imbalance in free radicals (unstable oxygen molecules) and antioxidants in the liver. Oxidative stress can lead to what\u2019s known as apoptosis &#8211; programmed cell death. <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4661801\/\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[5]<\/span><\/a><\/p>\n<p><span style=\"font-weight: 400;\">By using a mouse model, researchers were able to ascertain that injections of UC-MSC-Exo inhibited apoptosis in liver injury, reversing the fallout from oxidative stress. <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5857330\/\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[6]<\/span><\/a><\/p>\n<p>&nbsp;<\/p>\n<p><b>Promote heart repair<\/b><\/p>\n<p><span style=\"font-weight: 400;\">There have been several studies indicating that exosomes derived from umbilical cord mesenchymal stem cells have the ability to reduce myocardial injury &#8211; damage to heart tissue.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A study from 2018 showed that UC-MSC-Exo may have the ability to promote the expression of Smad7, a protein which aids in myocardial protection by blocking certain signalling pathways.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Researchers believed this could be the mechanism underlying exosomes\u2019 ability to inhibit apoptosis, promote angiogenesis (the formation of new blood vessels) and improve cardiac function following acute myocardial infarction. <\/span><a href=\"https:\/\/www.spandidos-publications.com\/ijmm\/41\/5\/3063\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[7]<\/span><\/a> <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/10.1155\/2015\/761643\" target=\"_blank\" rel=\"noreferrer noopener\"><span style=\"font-weight: 400;\">[8]<\/span><\/a><\/p>\n<p>&nbsp;<\/p>\n<p><b>Rejuvenate older stem cells<\/b><\/p>\n<p><span style=\"font-weight: 400;\">An especially intriguing study from 2020 also showed that exosomes derived from umbilical cord mesenchymal stem cells could help in myocardial repair by rejuvenating the activity and function of mesenchymal stem cells in older patients.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Because heart conditions mainly affect older patients, the risk of immunogenicity associated with allogeneic stem cell transplants in helping to treat heart damage is a potential risk. This leaves autologous stem cell transplants &#8211; using stem cells from the patient themselves &#8211; as an available treatment option. But when feeling down persists for weeks, or even months, it can be a sign of a mental health condition, like depression.<\/p><p>Although today people are well versed and more understanding of depression, some sufferers still find it difficult to find treatments.<\/p><h3>What is depression?<\/h3><p>Depression is a common mental health condition. It is characterised by low mood, feelings of hopelessness, losing interest in things which previously brought a sense of enjoyment, and feeling tearful.<\/p><p>Depression can also manifest in physical ways. Sleeping poorly, feeling tired, experiencing aches, pains, a diminished appetite, and decreased libido can all be impacts of depression. The symptoms range from mild to severe. In mild cases, a persistent low mood may be the cause, while severe cases may result in suicidal feelings or a desire to self harm <a href=\"https:\/\/www.nhs.uk\/mental-health\/conditions\/depression-in-adults\/overview\/\">[1]<\/a>.<\/p><p>Sometimes the causes of depression can be obvious, such as a bereavement or a traumatic event. Other times there might be no known cause, although family history could be the reason. Whatever the reason, once a person has identified they may be depressed, they should go and see their GP.<\/p><h3>What treatments are available?<\/h3><p>There are a plethora of treatments for depression that a doctor can prescribe, from talking therapies to pharmaceutical medications to exercise and more. Sometimes the GP may recommend one treatment or a combination of treatments but they will help support patients and signpost them to services which can bolster their support network.<\/p><p>The good news is that depression is among the most treatable mental health disorders.\u00a0 Between 80% and 90% percent of people with depression eventually respond well to treatment. Almost all patients gain some relief from their symptoms <a href=\"https:\/\/www.psychiatry.org\/patients-families\/depression\/what-is-depression#:~:text=Depression%20is%20among%20the%20most,some%20relief%20from%20their%20symptoms.\">[2]<\/a>.<\/p><h3>Treatment Resistant Depression<\/h3><p>For those with major depressive disorder, however, treatment can be harder to find. With up to one third of adults with the condition failing to respond to treatment, new avenues and treatments are being sought.<\/p><p>\u201cAlthough there is some disagreement as to how to define treatment-resistant depression, a patient is generally considered to have it if the individual hasn\u2019t responded to adequate doses of two different antidepressants taken for a sufficient duration of time, which is usually six weeks,\u201d explains Jaskaran Singh, M.D., Senior Director of Neuroscience, Janssen Pharmaceuticals <a href=\"https:\/\/www.jnj.com\/health-and-wellness\/4-facts-about-treatment-resistant-depression\">[3]<\/a>.<\/p><p>The theory that depression may have causes currently unknown by scientists is one theory researchers are exploring to try and combat resistance to treatment.<\/p><p>\u201cOne of the more modern theories is that depression creates inflammation in the brain, or that inflammation in the brain creates depression. Traditional antidepressants only affect neurotransmitters, so this may be why some patients don\u2019t respond to them.\u201d\u00a0 says Alexander Papp, M.D., a psychiatrist at UC San Diego Health <a href=\"https:\/\/www.jnj.com\/health-and-wellness\/4-facts-about-treatment-resistant-depression\">[4]<\/a>.<\/p><p>Whether or not this is the case, researchers are still trying to find solutions to Treatment Resistant Depression.<\/p><h3>The Potential of Cord Blood Therapy<\/h3><p>One study using umbilical cord blood cells delivered promising results. The study administered concentrated umbilical cord blood cells at a dose of 250 million cells via weekly injections over 4 weeks with 1 week intervals, to women who suffered with Treatment Resistant Depression.\u00a0 The results were tested against a control group who received a placebo.<\/p><p>The study found that the therapeutic potential of umbilical cord blood cell concentrate can be used to overcome treatment resistance formed in depressive patients <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7087609\/\">[5]<\/a>. Other studies have also found that cellular therapies may have a role in combating Treatment Resistant Depression <a href=\"https:\/\/www.mdpi.com\/1999-4923\/15\/3\/814\">[6]<\/a> <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC8474995\/\">[7]<\/a>.<\/p><p>These studies give hope for finding new treatments.
