The therapy is based on cells derived from allogeneic (donor) umbilical cord blood; it is the very first such treatment to gain FDA approval.<a href=\"https:\/\/www.businesswire.com\/news\/home\/20250129313138\/en\/VectorBuilder-Powers-FDA-IND-Approval-of-World%E2%80%99s-First-Umbilical-Cord-Blood-Derived-Allogeneic-CAR-T-Product\" target=\"_blank\" rel=\"noopener\">[2]<\/a><\/p>\n<h3 class=\"ai-optimize-44\"><span style=\"font-weight: 400;\">What is acute lymphoblastic leukemia?\u00a0<\/span><\/h3>\n<p class=\"ai-optimize-45\" style=\"font-weight: 400;\">Acute lymphoblastic leukemia, or ALL, is a type of blood cancer that starts in the bone marrow. It affects white blood cells (lymphocytes).<\/p>\n<p class=\"ai-optimize-46\" style=\"font-weight: 400;\">In ALL, the bone marrow makes too many of the cells from which lymphocytes derive (lymphoblasts). These lymphoblasts grow and divide quickly; however, they are not fully developed and cannot turn into healthy lymphocytes.<\/p>\n<p class=\"ai-optimize-47\" style=\"font-weight: 400;\">As a result, lymphoblasts build up in the bone marrow and stop all types of healthy blood cells from developing. They may also spread into other parts of the body, including lymph nodes, the liver and spleen, and the brain and spinal cord.<a href=\"https:\/\/www.cancerresearchuk.org\/about-cancer\/acute-lymphoblastic-leukaemia-all\/about\" target=\"_blank\" rel=\"noopener\">[3]<\/a><\/p>\n<p class=\"ai-optimize-48\" style=\"font-weight: 400;\">The lack of healthy white blood cells means ALL patients are at a higher risk of infection. ALL can also cause anemia through the lack of red blood cells, as well as bleeding problems due to lower platelet numbers.<a href=\"https:\/\/www.cancerresearchuk.org\/about-cancer\/childrens-cancer\/acute-lymphoblastic-leukaemia\/about\" target=\"_blank\" rel=\"noopener\">[4]<\/a><\/p>\n<p class=\"ai-optimize-49\" style=\"font-weight: 400;\">Leukemia is the most common type of cancer which affects children; of all types of leukemia, ALL is the most commonly diagnosed in children.<a href=\"https:\/\/www.cancerresearchuk.org\/about-cancer\/childrens-cancer\/acute-lymphoblastic-leukaemia\/about\" target=\"_blank\" rel=\"noopener\">[4]<\/a> It can also affect adults, although this is less common.<a href=\"https:\/\/www.cancerresearchuk.org\/about-cancer\/acute-lymphoblastic-leukaemia-all\/about\" target=\"_blank\" rel=\"noopener\">[3]<\/a><\/p>\n<h3 class=\"ai-optimize-50\"><span style=\"font-weight: 400;\">What is CAR-T cell therapy?<\/span><\/h3>\n<p class=\"ai-optimize-51\" style=\"font-weight: 400;\">CAR-T therapy is a type of treatment which uses T cells to fight cancerous cells. The T cells are genetically modified using a viral vector to add a specific protein on their surface, called a chimeric antigen receptor (CAR). This addition makes them able to recognize cancerous cells and destroy them.<a href=\"https:\/\/www.cancerresearchuk.org\/about-cancer\/treatment\/immunotherapy\/types\/CAR-T-cell-therapy\" target=\"_blank\" rel=\"noopener\">[5]<\/a><\/p>\n<p class=\"ai-optimize-52\" style=\"font-weight: 400;\">In standard CAR-T therapy, the T cells used are the patient\u2019s own, extracted from the blood through a process called apheresis. After extraction, they are taken to a specialized lab to be modified. However, this process is very time-consuming, typically requiring three to five weeks for manufacturing and quality checking. This can be a problem in the case of ALL, as the disease is fast-progressing and could worsen before the CAR-T cells are ready to be reinfused.<a href=\"https:\/\/www.lls.org\/treatment\/types-treatment\/immunotherapy\/chimeric-antigen-receptor-car-t-cell-therapy\" target=\"_blank\" rel=\"noopener\">[6]<\/a><\/p>\n<h3 class=\"ai-optimize-53\"><span style=\"font-weight: 400;\">What makes this new therapy different?<\/span><\/h3>\n<p class=\"ai-optimize-54\" style=\"font-weight: 400;\">The new therapy uses T cells derived from allogeneic (someone else\u2019s) cord blood as a starting point instead. These cells are in a more naive state, meaning they have a higher ability for self-renewal and lower immunogenicity. This means a lower chance to provoke an immune response when used in treatment. As a result, they can be developed into an \u2018off-the-shelf\u2019 CAR-T therapy that is ready when needed and does not require a long wait.<a href=\"https:\/\/www.businesswire.com\/news\/home\/20250129313138\/en\/VectorBuilder-Powers-FDA-IND-Approval-of-World%E2%80%99s-First-Umbilical-Cord-Blood-Derived-Allogeneic-CAR-T-Product\" target=\"_blank\" rel=\"noopener\">[2]<\/a> Following positive results from investigator-initiated trials,<a href=\"https:\/\/www.lemanbio.com\/en\/news\/197.html\" target=\"_blank\" rel=\"noopener\">[1]<\/a> the FDA has now approved the therapy for clinical trials in the United States.<a href=\"https:\/\/www.businesswire.com\/news\/home\/20250129313138\/en\/VectorBuilder-Powers-FDA-IND-Approval-of-World%E2%80%99s-First-Umbilical-Cord-Blood-Derived-Allogeneic-CAR-T-Product\" target=\"_blank\" rel=\"noopener\">[2]<\/a> Furthermore, researchers in China are also testing the therapy in a clinical trial for the treatment of lupus.<a href=\"https:\/\/mp.weixin.qq.com\/s\/t7tQTUwpl5vmr78cSRXGhQ\" target=\"_blank\" rel=\"noopener\">[7]<\/a><a href=\"https:\/\/mp.weixin.qq.com\/s\/rVx9bVuU3-2pKio_LbO7ZQ\" target=\"_blank\" rel=\"noopener\">[8]<\/a><\/p>\n<p class=\"ai-optimize-55\" style=\"font-weight: 400;\">A similar therapy, also based on T cells derived from donor cord blood, is being developed at the Fondazione Tettamanti research center in Monza, Italy. Using a special DNA sequence called a transposon, scientists turn the T cells into CAR-enabled cytokine-induced killer cells (CARCIK), making them able to destroy cancer cells. An initial set of clinical trials aimed at determining the feasibility and safety of this treatment showed positive results. Researchers are now continuing to test the therapy in clinical trials for ALL as well as non-Hodgkin lymphoma, and in vitro and animal models for acute myeloid leukemia.<a href=\"https:\/\/www.ilsole24ore.com\/art\/dal-cordone-ombelicale-cellule-modificate-trattare-leucemia-AGD2eidC\" target=\"_blank\" rel=\"noopener\">[9]<\/a><\/p>\n<h3 class=\"ai-optimize-56\"><span style=\"font-weight: 400;\">The benefits of using cord blood for treatments<\/span><\/h3>\n<p class=\"ai-optimize-57\" style=\"font-weight: 400;\">Being able to derive T cells for CAR treatment from cord blood could pave the way towards more targeted therapies in the future, says Sarah Tettamanti, a researcher at Fondazione Tettamanti. These therapies could be developed from banked cord blood compatible with each specific patient.<a href=\"https:\/\/www.ilsole24ore.com\/art\/dal-cordone-ombelicale-cellule-modificate-trattare-leucemia-AGD2eidC\" target=\"_blank\" rel=\"noopener\">[9]<\/a><\/p>\n<p class=\"ai-optimize-58\" style=\"font-weight: 400;\">For treatments such as these, it could be essential to have ready access to a compatible donor match. Public cord blood banks are available in the UK, US and worldwide. However, because they rely on donated cords, there is no guarantee that a match will be found when needed.<\/p>\n<p class=\"ai-optimize-59\" style=\"font-weight: 400;\">Conversely, if you store your baby\u2019s cord blood privately at birth, they will always have access to their own perfect match if they ever need it. There is also a 25% chance for the cord blood to be a perfect match for a sibling and 50% chance of a partial match. But when feeling down persists for weeks, or even months, it can be a sign of a mental health condition, like depression.<\/p><p>Although today people are well versed and more understanding of depression, some sufferers still find it difficult to find treatments.<\/p><h3>What is depression?<\/h3><p>Depression is a common mental health condition. It is characterised by low mood, feelings of hopelessness, losing interest in things which previously brought a sense of enjoyment, and feeling tearful.<\/p><p>Depression can also manifest in physical ways. Sleeping poorly, feeling tired, experiencing aches, pains, a diminished appetite, and decreased libido can all be impacts of depression. The symptoms range from mild to severe. In mild cases, a persistent low mood may be the cause, while severe cases may result in suicidal feelings or a desire to self harm <a href=\"https:\/\/www.nhs.uk\/mental-health\/conditions\/depression-in-adults\/overview\/\">[1]<\/a>.<\/p><p>Sometimes the causes of depression can be obvious, such as a bereavement or a traumatic event. Other times there might be no known cause, although family history could be the reason. Whatever the reason, once a person has identified they may be depressed, they should go and see their GP.<\/p><h3>What treatments are available?<\/h3><p>There are a plethora of treatments for depression that a doctor can prescribe, from talking therapies to pharmaceutical medications to exercise and more. Sometimes the GP may recommend one treatment or a combination of treatments but they will help support patients and signpost them to services which can bolster their support network.<\/p><p>The good news is that depression is among the most treatable mental health disorders.\u00a0 Between 80% and 90% percent of people with depression eventually respond well to treatment. Almost all patients gain some relief from their symptoms <a href=\"https:\/\/www.psychiatry.org\/patients-families\/depression\/what-is-depression#:~:text=Depression%20is%20among%20the%20most,some%20relief%20from%20their%20symptoms.\">[2]<\/a>.<\/p><h3>Treatment Resistant Depression<\/h3><p>For those with major depressive disorder, however, treatment can be harder to find. With up to one third of adults with the condition failing to respond to treatment, new avenues and treatments are being sought.<\/p><p>\u201cAlthough there is some disagreement as to how to define treatment-resistant depression, a patient is generally considered to have it if the individual hasn\u2019t responded to adequate doses of two different antidepressants taken for a sufficient duration of time, which is usually six weeks,\u201d explains Jaskaran Singh, M.D., Senior Director of Neuroscience, Janssen Pharmaceuticals <a href=\"https:\/\/www.jnj.com\/health-and-wellness\/4-facts-about-treatment-resistant-depression\">[3]<\/a>.<\/p><p>The theory that depression may have causes currently unknown by scientists is one theory researchers are exploring to try and combat resistance to treatment.<\/p><p>\u201cOne of the more modern theories is that depression creates inflammation in the brain, or that inflammation in the brain creates depression. Traditional antidepressants only affect neurotransmitters, so this may be why some patients don\u2019t respond to them.\u201d\u00a0 says Alexander Papp, M.D., a psychiatrist at UC San Diego Health <a href=\"https:\/\/www.jnj.com\/health-and-wellness\/4-facts-about-treatment-resistant-depression\">[4]<\/a>.<\/p><p>Whether or not this is the case, researchers are still trying to find solutions to Treatment Resistant Depression.<\/p><h3>The Potential of Cord Blood Therapy<\/h3><p>One study using umbilical cord blood cells delivered promising results. The study administered concentrated umbilical cord blood cells at a dose of 250 million cells via weekly injections over 4 weeks with 1 week intervals, to women who suffered with Treatment Resistant Depression.\u00a0 The results were tested against a control group who received a placebo.<\/p><p>The study found that the therapeutic potential of umbilical cord blood cell concentrate can be used to overcome treatment resistance formed in depressive patients <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7087609\/\">[5]<\/a>. Other studies have also found that cellular therapies may have a role in combating Treatment Resistant Depression <a href=\"https:\/\/www.mdpi.com\/1999-4923\/15\/3\/814\">[6]<\/a> <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC8474995\/\">[7]<\/a>.<\/p><p>These studies give hope for finding new treatments.
