The therapy has shown promise in relieving symptoms and reducing disease activity in children with a severe form of EB.<a href=\"https:\/\/epidermolysisbullosanews.com\/news\/stem-cell-therapy-reduces-itch-pain-rdeb-children-trial\/\" target=\"_blank\" rel=\"noopener\">[1]<\/a><\/p>\n<h3><span style=\"font-weight: 400;\">What is epidermolysis bullosa?\u00a0<\/span><\/h3>\n<p style=\"font-weight: 400;\">EB is a group of inherited genetic disorders that results in easy blistering of the skin and, in many cases, mucous membranes, such as the lining inside the mouth. With EB, painful blisters or open sores will form with even the slightest friction, even from seemingly trivial things such as clothes rubbing against the skin.<\/p>\n<p style=\"font-weight: 400;\">This extreme skin weakness means that everyday things most of us don\u2019t think about can be very difficult and painful for people with EB. What\u2019s more, there is currently no cure available for EB.<a href=\"https:\/\/www.nhs.uk\/conditions\/epidermolysis-bullosa\/treatment\/\" target=\"_blank\" rel=\"noopener\">[2]<\/a> Because of this, patients and their families have to be constantly careful, avoiding anything that might hurt the skin as much as possible. Dealing with the blisters and sores that do happen, to avoid them spreading further or causing infections, is also a painful, time-consuming daily task.<a href=\"https:\/\/epidermolysisbullosanews.com\/what-is-epidermolysis-bullosa\/\" target=\"_blank\" rel=\"noopener\">[3]<\/a> The blisters need to be lanced and dressed, in order to avoid them causing larger, painful wounds that take a long time to heal.<a href=\"https:\/\/www.nhs.uk\/conditions\/epidermolysis-bullosa\/treatment\/\" target=\"_blank\" rel=\"noopener\">[2]<\/a> Itching is also very common, with patients describing an unbearable, constant itch. Moreover, relief from the itching is difficult, as scratching, too, can tear the skin, create blisters and make existing wounds worse.<a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC4315706\/\" target=\"_blank\" rel=\"noopener\">[4]<\/a><\/p>\n<p style=\"font-weight: 400;\">Depending on the type of EB and severity of the disease, sufferers may be left with deformities from scar tissue, such as fused fingers or toes, and limited motion. If blisters form inside the mouth or in the oesophagus, eating can also become an issue. Skin cancer and sepsis are potentially lethal consequences.<a href=\"https:\/\/academic.oup.com\/ced\/advance-article\/doi\/10.1093\/ced\/llaf009\/7985666?login=false\" target=\"_blank\" rel=\"noopener\">[5]<\/a><\/p>\n<p style=\"font-weight: 400;\">There are four main types of EB, varying in severity: EB simplex, dystrophic EB, junctional EB and Kindler syndrome.<a href=\"https:\/\/epidermolysisbullosanews.com\/what-is-epidermolysis-bullosa\/\" target=\"_blank\" rel=\"noopener\">[3]<\/a> Among these, recessive dystrophic EB (RDEB) is one of the most severe types, and in fact one of the most severe hereditary genetic disorders as a whole<a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC4445895\/\" target=\"_blank\" rel=\"noopener\">[6]<\/a>. In other words, as the name implies, it is recessively inherited, with two parents who are both asymptomatic carriers of the disease having a one in four chance of having a child affected by it.<a href=\"https:\/\/www.debra.org.uk\/get-support\/eb-support-and-resources\/about-eb-debra-uk\/dystrophic-epidermolysis-bullosa\/\" target=\"_blank\" rel=\"noopener\">[7]<\/a> Patients with this type of EB also have a higher chance of developing skin cancer, and their life expectancy is just age 30.<a href=\"https:\/\/www.ebresearch.org\/dystrophic.html\" target=\"_blank\" rel=\"noopener\">[8]<\/a><\/p>\n<h3><span style=\"font-weight: 400;\">Trial structure and objectives<\/span><\/h3>\n<p style=\"font-weight: 400;\">The study on the cord tissue stem cell therapy for EB, called MissionEB, focused on children with the two most severe types of RDEB, designated \u201cintermediate\u201d and \u201csevere\u201d in disease classification. It was led by the EB team at Great Ormond Street Hospital<a href=\"https:\/\/epidermolysisbullosanews.com\/news\/stem-cell-therapy-reduces-itch-pain-rdeb-children-trial\/\" target=\"_blank\" rel=\"noopener\">[1]<\/a>, one of the four centres of excellence for EB treatment in the UK<a href=\"https:\/\/www.debra.org.uk\/get-support\/eb-support-and-resources\/eb-professionals\/specialist-eb-healthcare\/\" target=\"_blank\" rel=\"noopener\">[9]<\/a>.<\/p>\n<p style=\"font-weight: 400;\">A prior small-scale clinical trial had found benefits from a therapy consisting of mesenchymal stem cells (MSCs) derived from bone marrow, with effects lasting 3-6 months.<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0022202X15390084\" target=\"_blank\" rel=\"noopener\">[10]<\/a> Following this, researchers posited that MSCs derived from umbilical cord tissue (UC-MSCs) could prove more effective.<a href=\"https:\/\/cure-eb.org\/research-portfolio\/mission-eb\/\" target=\"_blank\" rel=\"noopener\">[11]<\/a> Therefore, the study aimed at testing whether repeated doses of UC-MSCs could be a safe and effective treatment for severe RDEB.<\/p>\n<p style=\"font-weight: 400;\">The study enrolled 30 children, and was a randomised, placebo controlled, double blinded cross-over trial. In other words, patients were randomly assigned to the treatment or placebo group. Furthermore, neither the patients nor the researchers were aware which study group each patient was in. After an initial assessment at 9 months, groups were then switched, with the original treatment group receiving placebo and vice versa.<a href=\"https:\/\/cure-eb.org\/research-portfolio\/mission-eb\/\" target=\"_blank\" rel=\"noopener\">[11]<\/a><a href=\"https:\/\/www.isrctn.com\/ISRCTN14409785\" target=\"_blank\" rel=\"noopener\">[12]<\/a><\/p>\n<p style=\"font-weight: 400;\">Preliminary results of the study show that the treatment improved patients\u2019 quality of life, reducing itch and pain. The range of improvements was broader for patients with less severe RDEB; children younger than 10 also saw improved skin integrity and reduced disease activity. In interviews, both the children and their care givers correctly identified whether they were receiving the treatment or the placebo.<a href=\"https:\/\/cure-eb.org\/research-portfolio\/mission-eb\/\" target=\"_blank\" rel=\"noopener\">[11]<\/a><a href=\"https:\/\/epidermolysisbullosanews.com\/news\/stem-cell-therapy-reduces-itch-pain-rdeb-children-trial\/\" target=\"_blank\" rel=\"noopener\">[1]<\/a><\/p>\n<p style=\"font-weight: 400;\">An open-label extension study is now ongoing. In this study, patients who completed the original MissionEB trial will receive three cycles of the treatment, at zero, four, and eight months. The severity of their disease will then be assessed at four, eight, and twelve months.<a href=\"https:\/\/epidermolysisbullosanews.com\/news\/stem-cell-therapy-reduces-itch-pain-rdeb-children-trial\/\" target=\"_blank\" rel=\"noopener\">[1]<\/a><\/p>\n<h3><span style=\"font-weight: 400;\">The future of cord tissue stem cell therapy for EB<\/span><\/h3>\n<p style=\"font-weight: 400;\">Following an agreement with Great Ormond Street Hospital for use of the data, biotechnology company Inmune Bio is marketing the treatment as CORDStrom. But when feeling down persists for weeks, or even months, it can be a sign of a mental health condition, like depression.<\/p><p>Although today people are well versed and more understanding of depression, some sufferers still find it difficult to find treatments.<\/p><h3>What is depression?<\/h3><p>Depression is a common mental health condition. It is characterised by low mood, feelings of hopelessness, losing interest in things which previously brought a sense of enjoyment, and feeling tearful.<\/p><p>Depression can also manifest in physical ways. Sleeping poorly, feeling tired, experiencing aches, pains, a diminished appetite, and decreased libido can all be impacts of depression. The symptoms range from mild to severe. In mild cases, a persistent low mood may be the cause, while severe cases may result in suicidal feelings or a desire to self harm <a href=\"https:\/\/www.nhs.uk\/mental-health\/conditions\/depression-in-adults\/overview\/\">[1]<\/a>.<\/p><p>Sometimes the causes of depression can be obvious, such as a bereavement or a traumatic event. Other times there might be no known cause, although family history could be the reason. Whatever the reason, once a person has identified they may be depressed, they should go and see their GP.<\/p><h3>What treatments are available?<\/h3><p>There are a plethora of treatments for depression that a doctor can prescribe, from talking therapies to pharmaceutical medications to exercise and more. Sometimes the GP may recommend one treatment or a combination of treatments but they will help support patients and signpost them to services which can bolster their support network.<\/p><p>The good news is that depression is among the most treatable mental health disorders.\u00a0 Between 80% and 90% percent of people with depression eventually respond well to treatment. Almost all patients gain some relief from their symptoms <a href=\"https:\/\/www.psychiatry.org\/patients-families\/depression\/what-is-depression#:~:text=Depression%20is%20among%20the%20most,some%20relief%20from%20their%20symptoms.\">[2]<\/a>.<\/p><h3>Treatment Resistant Depression<\/h3><p>For those with major depressive disorder, however, treatment can be harder to find. With up to one third of adults with the condition failing to respond to treatment, new avenues and treatments are being sought.<\/p><p>\u201cAlthough there is some disagreement as to how to define treatment-resistant depression, a patient is generally considered to have it if the individual hasn\u2019t responded to adequate doses of two different antidepressants taken for a sufficient duration of time, which is usually six weeks,\u201d explains Jaskaran Singh, M.D., Senior Director of Neuroscience, Janssen Pharmaceuticals <a href=\"https:\/\/www.jnj.com\/health-and-wellness\/4-facts-about-treatment-resistant-depression\">[3]<\/a>.<\/p><p>The theory that depression may have causes currently unknown by scientists is one theory researchers are exploring to try and combat resistance to treatment.<\/p><p>\u201cOne of the more modern theories is that depression creates inflammation in the brain, or that inflammation in the brain creates depression. Traditional antidepressants only affect neurotransmitters, so this may be why some patients don\u2019t respond to them.\u201d\u00a0 says Alexander Papp, M.D., a psychiatrist at UC San Diego Health <a href=\"https:\/\/www.jnj.com\/health-and-wellness\/4-facts-about-treatment-resistant-depression\">[4]<\/a>.<\/p><p>Whether or not this is the case, researchers are still trying to find solutions to Treatment Resistant Depression.<\/p><h3>The Potential of Cord Blood Therapy<\/h3><p>One study using umbilical cord blood cells delivered promising results. The study administered concentrated umbilical cord blood cells at a dose of 250 million cells via weekly injections over 4 weeks with 1 week intervals, to women who suffered with Treatment Resistant Depression.\u00a0 The results were tested against a control group who received a placebo.<\/p><p>The study found that the therapeutic potential of umbilical cord blood cell concentrate can be used to overcome treatment resistance formed in depressive patients <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7087609\/\">[5]<\/a>. Other studies have also found that cellular therapies may have a role in combating Treatment Resistant Depression <a href=\"https:\/\/www.mdpi.com\/1999-4923\/15\/3\/814\">[6]<\/a> <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC8474995\/\">[7]<\/a>.<\/p><p>These studies give hope for finding new treatments.
