{"id":38547,"date":"2025-07-07T13:14:39","date_gmt":"2025-07-07T13:14:39","guid":{"rendered":"https:\/\/cells4life.com\/us\/?p=38547"},"modified":"2025-09-29T16:06:08","modified_gmt":"2025-09-29T16:06:08","slug":"could-stem-cells-treat-acute-on-chronic-liver-failure","status":"publish","type":"post","link":"https:\/\/cells4life.com\/us\/2025\/07\/could-stem-cells-treat-acute-on-chronic-liver-failure\/","title":{"rendered":"Could Stem Cells Help Treat Acute-on-Chronic Liver Failure?"},"content":{"rendered":"[et_pb_section fb_built=&#8221;1&#8243; _builder_version=&#8221;4.16&#8243; global_colors_info=&#8221;{}&#8221;][et_pb_row _builder_version=&#8221;4.16&#8243; background_size=&#8221;initial&#8221; background_position=&#8221;top_left&#8221; background_repeat=&#8221;repeat&#8221; global_colors_info=&#8221;{}&#8221;][et_pb_column type=&#8221;4_4&#8243; _builder_version=&#8221;4.16&#8243; custom_padding=&#8221;|||&#8221; global_colors_info=&#8221;{}&#8221; custom_padding__hover=&#8221;|||&#8221;][et_pb_text _builder_version=&#8221;4.27.4&#8243; background_size=&#8221;initial&#8221; background_position=&#8221;top_left&#8221; background_repeat=&#8221;repeat&#8221; hover_enabled=&#8221;0&#8243; locked=&#8221;off&#8221; global_colors_info=&#8221;{}&#8221; sticky_enabled=&#8221;0&#8243;]\n<p class=\"ai-optimize-45\" style=\"font-weight: 400;\">The liver is a remarkably resilient organ &#8211; the only organ in the body capable of regenerating itself when damaged.<a href=\"https:\/\/britishlivertrust.org.uk\/information-and-support\/liver-health-2\/abouttheliver\/#repair\" target=\"_blank\" rel=\"noopener\">[1]<\/a> Damage to the liver does, however, build up over time, eventually leading first to chronic (long-term) liver disease and then, inevitably, to liver failure.<a href=\"https:\/\/www.hopkinsmedicine.org\/health\/conditions-and-diseases\/chronic-liver-disease-cirrhosis\" target=\"_blank\" rel=\"noopener\">[2]<\/a> If liver failure occurs suddenly and also involves the failure of other organs, it is called acute-on-chronic liver failure (ACLF).<a href=\"https:\/\/link.springer.com\/article\/10.1186\/s13287-025-04303-8\" target=\"_blank\" rel=\"noopener\">[3]<\/a><\/p>\n<h3 class=\"ai-optimize-46\"><span style=\"font-weight: 400;\">A lack of treatment options<\/span><\/h3>\n<p class=\"ai-optimize-47\" style=\"font-weight: 400;\">The odds of ACLF occurring increase as a patient\u2019s chronic liver disease worsens, ranging from around 7%<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/30615211\/\" target=\"_blank\" rel=\"noopener\">[4]<\/a> in the early stages of cirrhosis (which itself is the final stage of chronic liver disease) to about 35%<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33436495\/\" target=\"_blank\" rel=\"noopener\">[5]<\/a> in the end stage. It can, however, even happen in very early chronic liver disease, although this is quite rare.<a href=\"https:\/\/www.frontiersin.org\/journals\/medicine\/articles\/10.3389\/fmed.2021.752875\/full\" target=\"_blank\" rel=\"noopener\">[6]<\/a><\/p>\n<p class=\"ai-optimize-48\" style=\"font-weight: 400;\">ACLF is generally triggered by something negative happening to the liver, such as drinking alcohol, bacterial infections, sepsis, or a hepatitis flare<a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC7539713\/\" target=\"_blank\" rel=\"noopener\">[7]<\/a><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK499902\/\" target=\"_blank\" rel=\"noopener\">[8]<\/a>. In up to 40-50% of cases, though, the trigger is never conclusively identified.<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK499902\/\" target=\"_blank\" rel=\"noopener\">[8]<\/a> Whatever the cause, the result is an uncontrolled, intense systemic inflammation, which then leads to organ damage and failure. This, in turn, worsens the inflammation, leading to a vicious cycle.<a href=\"https:\/\/link.springer.com\/article\/10.1186\/s13287-025-04303-8\" target=\"_blank\" rel=\"noopener\">[3]<\/a><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC7652714\/\" target=\"_blank\" rel=\"noopener\">[9]<\/a> Depending on the severity of the organ failure, 28-day mortality for ACLF can vary from around 18% to as high as nearly 89%.<a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC7539713\/\" target=\"_blank\" rel=\"noopener\">[7]<\/a><\/p>\n<p class=\"ai-optimize-49\" style=\"font-weight: 400;\">Treatment options for ACLF are limited to identifying and treating the triggering event, as well as supporting failing organs and managing complications while helping the body to recover.<a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC7539713\/\" target=\"_blank\" rel=\"noopener\">[7]<\/a><a href=\"https:\/\/www.aasld.org\/liver-fellow-network\/core-series\/clinical-pearls\/management-acute-chronic-liver-failure\" target=\"_blank\" rel=\"noopener\">[10]<\/a> A liver transplant is the only effective treatment, with survival rates for patients with severe ACLF who receive a transplant reaching nearly 80%.<a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC7539713\/\" target=\"_blank\" rel=\"noopener\">[7]<\/a> However, not every patient may be eligible for a transplant<a href=\"https:\/\/www.aasld.org\/liver-fellow-network\/core-series\/clinical-pearls\/management-acute-chronic-liver-failure\" target=\"_blank\" rel=\"noopener\">[10]<\/a> or, for that matter, find a match in time, given how quickly ACLF progresses<a href=\"https:\/\/link.springer.com\/article\/10.1186\/s13287-025-04303-8\" target=\"_blank\" rel=\"noopener\">[3]<\/a><a href=\"https:\/\/britishlivertrust.org.uk\/information-and-support\/liver-conditions\/cirrhosis\/#aclf\" target=\"_blank\" rel=\"noopener\">[11]<\/a>. There is therefore an urgent need for a treatment for ACLF that does not rely on a liver transplant.<\/p>\n<h3 class=\"ai-optimize-50\"><span style=\"font-weight: 400;\">Addressing the root cause<\/span><\/h3>\n<p class=\"ai-optimize-51\" style=\"font-weight: 400;\">Because systemic inflammation is a key component of ACLF, an effective treatment must target this as well as the underlying liver damage. This is not always possible with currently available treatment options. Treating the trigger for the inflammation can make a difference,<a href=\"https:\/\/britishlivertrust.org.uk\/information-and-support\/liver-conditions\/cirrhosis\/#aclf\" target=\"_blank\" rel=\"noopener\">[11]<\/a> but this cannot even always be conclusively identified. Moreover, the only available method to treat liver damage once it has reached the chronic stage is a transplant, which, as previously mentioned, is not always an option.<\/p>\n<p class=\"ai-optimize-52\" style=\"font-weight: 400;\">Mesenchymal stem cells (MSCs) are powerful, tissue-forming stem cells, which can help in wound healing and tissue regeneration. MSCs also have a strong potential for regulating the body\u2019s immune response and reducing inflammation. It is these properties that have drawn researchers\u2019 attention to their potential uses in the field of regenerative medicine,<a href=\"https:\/\/www.frontiersin.org\/journals\/medicine\/articles\/10.3389\/fmed.2021.752875\/full\" target=\"_blank\" rel=\"noopener\">[6]<\/a><a href=\"https:\/\/link.springer.com\/article\/10.1186\/s13287-022-03054-0\" target=\"_blank\" rel=\"noopener\">[12]<\/a> and that could make a difference in the treatment of ACLF by helping to regenerate the liver as well as reducing systemic inflammation.<a href=\"https:\/\/link.springer.com\/article\/10.1186\/s13287-025-04303-8\" target=\"_blank\" rel=\"noopener\">[3]<\/a><a href=\"https:\/\/www.frontiersin.org\/journals\/medicine\/articles\/10.3389\/fmed.2021.752875\/full\" target=\"_blank\" rel=\"noopener\">[6]<\/a><\/p>\n<p class=\"ai-optimize-53\" style=\"font-weight: 400;\">MSC therapy for ACLF is in the early stages of development; recently, a systematic review and meta-analysis of six clinical trials and one retrospective study was conducted, in order to better assess its safety and effectiveness.<a href=\"https:\/\/link.springer.com\/article\/10.1186\/s13287-025-04303-8\" target=\"_blank\" rel=\"noopener\">[3]<\/a><\/p>\n<h3 class=\"ai-optimize-54\"><span style=\"font-weight: 400;\">What were the results of the analysis?<\/span><\/h3>\n<p class=\"ai-optimize-55\" style=\"font-weight: 400;\">Across the board, MSCs proved to be a safe treatment for ACLF, with no adverse events or serious side effects being recorded in any of the individual studies. What\u2019s more, study results indicate the treatment could indeed prove to be effective.<\/p>\n<p class=\"ai-optimize-56\" style=\"font-weight: 400;\">Firstly, the review authors pooled data from a total of 363 patients to analyze scores on the Model for End-Stage Liver Disease (MELD) scale. This scale is used to assess the severity of chronic liver disease and determine the urgency of a liver transplant.<a href=\"https:\/\/my.clevelandclinic.org\/health\/diagnostics\/meld-score\" target=\"_blank\" rel=\"noopener\">[13]<\/a> MELD scores were found to have significantly decreased in patients who underwent the treatment, indicating an improvement in liver function and a better chance of survival.<\/p>\n<p class=\"ai-optimize-57\" style=\"font-weight: 400;\">Next, the authors analyzed the reported data on levels of albumin, a key protein made by the liver. Across the studies which reported them, albumin levels were shown to have increased, confirming the boost to liver function. Other markers of liver function also improved, although these were not found to have been statistically significant.<\/p>\n<p class=\"ai-optimize-58\" style=\"font-weight: 400;\">The authors also performed a time analysis, hoping to determine the best time to perform the treatment. They found that liver function improved at 4 and 24 weeks post-therapy in the treatment group when compared to the control group. However, there were no statistically significant differences between the two groups either earlier (2-week mark) or longer term. Thus, the authors suggest that the best result may be achieved by repeating the treatment once efficacy begins to wane.<\/p>\n<p class=\"ai-optimize-59\" style=\"font-weight: 400;\">Furthermore, the authors note that more research is needed to identify the best treatment route, as there currently isn\u2019t enough data to conclusively determine this. Scientists are also still investigating the best source of MSCs for treatment; of the many possible sources, most of the studies included in the review used MSCs from the umbilical cord.<\/p>\n<h3 class=\"ai-optimize-60\"><span style=\"font-weight: 400;\">The promise of stem cells<\/span><\/h3>\n<p class=\"ai-optimize-61\" style=\"font-weight: 400;\">Larger, more wide-ranging trials are needed to determine whether MSC treatment for ACLF is truly effective. Still, this research, along with ongoing studies into the use of stem cells for the treatment of other diseases and conditions, highlights the potential of regenerative medicine. people know what it\u2019s like to feel sadness. But when feeling down persists for weeks, or even months, it can be a sign of a mental health condition, like depression.<\/p><p>Although today people are well versed and more understanding of depression, some sufferers still find it difficult to find treatments.<\/p><h3>What is depression?<\/h3><p>Depression is a common mental health condition. It is characterised by low mood, feelings of hopelessness, losing interest in things which previously brought a sense of enjoyment, and feeling tearful.<\/p><p>Depression can also manifest in physical ways. Sleeping poorly, feeling tired, experiencing aches, pains, a diminished appetite, and decreased libido can all be impacts of depression. The symptoms range from mild to severe. In mild cases, a persistent low mood may be the cause, while severe cases may result in suicidal feelings or a desire to self harm <a href=\"https:\/\/www.nhs.uk\/mental-health\/conditions\/depression-in-adults\/overview\/\">[1]<\/a>.<\/p><p>Sometimes the causes of depression can be obvious, such as a bereavement or a traumatic event. Other times there might be no known cause, although family history could be the reason. Whatever the reason, once a person has identified they may be depressed, they should go and see their GP.<\/p><h3>What treatments are available?<\/h3><p>There are a plethora of treatments for depression that a doctor can prescribe, from talking therapies to pharmaceutical medications to exercise and more. Sometimes the GP may recommend one treatment or a combination of treatments but they will help support patients and signpost them to services which can bolster their support network.<\/p><p>The good news is that depression is among the most treatable mental health disorders.\u00a0 Between 80% and 90% percent of people with depression eventually respond well to treatment. Almost all patients gain some relief from their symptoms <a href=\"https:\/\/www.psychiatry.org\/patients-families\/depression\/what-is-depression#:~:text=Depression%20is%20among%20the%20most,some%20relief%20from%20their%20symptoms.\">[2]<\/a>.<\/p><h3>Treatment Resistant Depression<\/h3><p>For those with major depressive disorder, however, treatment can be harder to find. With up to one third of adults with the condition failing to respond to treatment, new avenues and treatments are being sought.<\/p><p>\u201cAlthough there is some disagreement as to how to define treatment-resistant depression, a patient is generally considered to have it if the individual hasn\u2019t responded to adequate doses of two different antidepressants taken for a sufficient duration of time, which is usually six weeks,\u201d explains Jaskaran Singh, M.D., Senior Director of Neuroscience, Janssen Pharmaceuticals <a href=\"https:\/\/www.jnj.com\/health-and-wellness\/4-facts-about-treatment-resistant-depression\">[3]<\/a>.<\/p><p>The theory that depression may have causes currently unknown by scientists is one theory researchers are exploring to try and combat resistance to treatment.<\/p><p>\u201cOne of the more modern theories is that depression creates inflammation in the brain, or that inflammation in the brain creates depression. Traditional antidepressants only affect neurotransmitters, so this may be why some patients don\u2019t respond to them.\u201d\u00a0 says Alexander Papp, M.D., a psychiatrist at UC San Diego Health <a href=\"https:\/\/www.jnj.com\/health-and-wellness\/4-facts-about-treatment-resistant-depression\">[4]<\/a>.<\/p><p>Whether or not this is the case, researchers are still trying to find solutions to Treatment Resistant Depression.<\/p><h3>The Potential of Cord Blood Therapy<\/h3><p>One study using umbilical cord blood cells delivered promising results. The study administered concentrated umbilical cord blood cells at a dose of 250 million cells via weekly injections over 4 weeks with 1 week intervals, to women who suffered with Treatment Resistant Depression.\u00a0 The results were tested against a control group who received a placebo.<\/p><p>The study found that the therapeutic potential of umbilical cord blood cell concentrate can be used to overcome treatment resistance formed in depressive patients <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7087609\/\">[5]<\/a>. Other studies have also found that cellular therapies may have a role in combating Treatment Resistant Depression <a href=\"https:\/\/www.mdpi.com\/1999-4923\/15\/3\/814\">[6]<\/a> <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC8474995\/\">[7]<\/a>.<\/p><p>These studies give hope for finding new treatments.
