This novel approach, tested in a first-of-its-kind clinical trial the results of which were published in <a href=\"https:\/\/www.nature.com\/articles\/s41467-025-60816-z\" target=\"_blank\" rel=\"noopener\">Nature Communications<\/a>, could potentially offer longer-lasting protection from cancer.<\/p>\n<h3 class=\"ai-optimize-15\"><span style=\"font-weight: 400;\">T-cell cancer therapies and their challenges<\/span><\/h3>\n<p class=\"ai-optimize-16\" style=\"font-weight: 400;\">T cells are a type of lymphocytes, white blood cells which are part of the immune system and move around the body finding and destroying abnormal cells to fight infection and disease. However, sometimes it can be difficult for T cells to tell the difference between cancerous cells and normal cells. This enables the cancerous cells to avoid destruction.<a href=\"https:\/\/www.cancerresearchuk.org\/about-cancer\/treatment\/targeted-cancer-drugs-immunotherapy\/car-t-cell-therapy\" target=\"_blank\" rel=\"noopener\">[1]<\/a><\/p>\n<p class=\"ai-optimize-17\" style=\"font-weight: 400;\">T-cell cancer therapies collect T cells from the patient\u2019s blood through a process called apheresis. Following this, the T cells are modified in the laboratory to make them better able to find and destroy cancerous cells. Then, the patient undergoes conditioning chemotherapy to weaken the immune system, in order to ensure the modified T cells have little competition from other T cells and can therefore be as effective as possible. Lastly, the modified T-cells are infused back into the patient.<a href=\"https:\/\/www.cancerresearchuk.org\/about-cancer\/treatment\/targeted-cancer-drugs-immunotherapy\/car-t-cell-therapy\" target=\"_blank\" rel=\"noopener\">[1]<\/a><\/p>\n<p class=\"ai-optimize-18\" style=\"font-weight: 400;\">One type of T-cell therapy is CAR T-cell therapy. This therapy modifies T cells by adding a protein, called a Chimeric Antigen Receptor (CAR), to their surface, enabling them to recognize a specific protein on the surface of cancer cells. CAR T-cell therapies are approved in the US to treat some types of leukemia and lymphoma, as well as multiple myeloma.<a href=\"https:\/\/www.cancerresearchuk.org\/about-cancer\/treatment\/targeted-cancer-drugs-immunotherapy\/car-t-cell-therapy\" target=\"_blank\" rel=\"noopener\">[1]<\/a><a href=\"https:\/\/www.cancer.org\/cancer\/managing-cancer\/treatment-types\/immunotherapy\/car-t-cell.html\" target=\"_blank\" rel=\"noopener\">[2]<\/a><\/p>\n<p class=\"ai-optimize-19\" style=\"font-weight: 400;\">These therapies have been successful in treating blood cancers where other types of treatment have failed. However, solid tumors, such as melanoma or breast cancer, are more difficult to target with cell therapies. This is due to the cancerous cells being more challenging to target without harming other tissues, as well as the need for cell therapies to get through body tissue to get to the tumor. Some cell therapies have recently been approved for use in clinical care in the United States, with more\u00a0 currently in development,<a href=\"https:\/\/www.uclh.nhs.uk\/news\/uclh-drive-forward-research-cell-therapies-solid-tumours\" target=\"_blank\" rel=\"noopener\">[3]<\/a> including the novel approach developed by the research team at UCLA.<\/p>\n<h3 class=\"ai-optimize-20\"><span style=\"font-weight: 400;\">How is the new therapy different?<\/span><\/h3>\n<p class=\"ai-optimize-21\" style=\"font-weight: 400;\">One of the problems with T cell therapy for solid tumors is that while the therapy works at first, resulting in shrinking of the tumor in a large proportion of patients, this result is not sustainable, with patients often relapsing within 6-12 months of the treatment. This is because the number of modified T cells in the body decreases over time. What\u2019s more, the ones that remain also become less effective. Thus, the need for a self-renewing source of these cells beyond the original infusion at point of treatment.<a href=\"https:\/\/www.nature.com\/articles\/s41467-025-60816-z\" target=\"_blank\" rel=\"noopener\">[4]<\/a><\/p>\n<p class=\"ai-optimize-22\" style=\"font-weight: 400;\">Indeed, the therapy developed by the research team at UCLA features a novel, two-pronged approach. First, patients underwent a process called mobilization, which involves using medication to stimulate the production of peripheral blood stem cells (PBSCs) and then collecting them through apheresis. The stem cells were then modified in the lab to enable them to give rise to T cells able to target a specific tumor marker (NY-ESO-1). Once the modified stem cells were ready and had been tested, T cells were collected from patients through a second apheresis and modified to be able to target the same NY-ESO-1 tumor marker. Following conditioning chemotherapy, both the modified stem cells and the modified T cells were reinfused into the patients.<\/p>\n<h3 class=\"ai-optimize-23\"><span style=\"font-weight: 400;\">What were the trial results?<\/span><\/h3>\n<p class=\"ai-optimize-24\" style=\"font-weight: 400;\">Five patients with relapsed or treatment-resistant metastatic sarcoma, a type of solid tumor, were recruited into the trial; two had to drop out of the study, and three ultimately received the treatment. Of the three, two had a positive response to the treatment, with a reduction in tumor size; the third showed no response, something which could possibly be attributed to the conditioning chemotherapy not being as effective as it could have been.<\/p>\n<p class=\"ai-optimize-25\" style=\"font-weight: 400;\">In one of the patients who had a positive response, the modified stem cells failed to engraft and produce any new T cells. Researchers attributed this to the fact that the process of modifying the stem cells, although successful, produced results that were at the lowest acceptable level for use in the clinical trial.<\/p>\n<p class=\"ai-optimize-26\" style=\"font-weight: 400;\">The last patient had successful engraftment of the stem cells, which began to produce new T cells. Unfortunately, she passed away during the study from a respiratory infection which arose as a complication of the conditioning chemotherapy.<\/p>\n<h3 class=\"ai-optimize-27\"><span style=\"font-weight: 400;\">The road towards a treatment for cancer<\/span><\/h3>\n<p class=\"ai-optimize-28\" style=\"font-weight: 400;\">This was a small, preliminary trial, limited to a handful of patients with advanced cancer. The results demonstrated that the two-pronged therapy approach is feasible and can work. However, the treatment needs further development and more extensive testing before it can be deemed ready for clinical use. What\u2019s more, it may not be suitable for all patients, given the dangers inherent to conditioning chemotherapy for those who are already very weak.<\/p>\n<p class=\"ai-optimize-29\" style=\"font-weight: 400;\">Still, there is no doubt that stem cells could hold the key to more effective treatments than those we already have, as well as cures for conditions and diseases currently considered incurable. Indeed, the researchers posit that variations on the approach tested in this trial might be used to target a wide variety of other diseases, such as HIV.<\/p>\n<p class=\"ai-optimize-30\" style=\"font-weight: 400;\">There are many possible sources of stem cells, such as from cord blood, fat, bone marrow, or peripheral blood, the latter of which was used in this particular trial. Some of these sources are always available throughout your lifetime, although they may be easier or harder to obtain stem cells from at the point of treatment. On the other hand, others, such as cord blood and other perinatal (birth-related) sources of stem cells, can only be obtained at specific times.<\/p>\n<p class=\"ai-optimize-31\" style=\"font-weight: 400;\">It is impossible to know, right now, which sources of stem cells could be most effective for therapies which are still in the early stages of development today. But when feeling down persists for weeks, or even months, it can be a sign of a mental health condition, like depression.<\/p><p>Although today people are well versed and more understanding of depression, some sufferers still find it difficult to find treatments.<\/p><h3>What is depression?<\/h3><p>Depression is a common mental health condition. It is characterised by low mood, feelings of hopelessness, losing interest in things which previously brought a sense of enjoyment, and feeling tearful.<\/p><p>Depression can also manifest in physical ways. Sleeping poorly, feeling tired, experiencing aches, pains, a diminished appetite, and decreased libido can all be impacts of depression. The symptoms range from mild to severe. In mild cases, a persistent low mood may be the cause, while severe cases may result in suicidal feelings or a desire to self harm <a href=\"https:\/\/www.nhs.uk\/mental-health\/conditions\/depression-in-adults\/overview\/\">[1]<\/a>.<\/p><p>Sometimes the causes of depression can be obvious, such as a bereavement or a traumatic event. Other times there might be no known cause, although family history could be the reason. Whatever the reason, once a person has identified they may be depressed, they should go and see their GP.<\/p><h3>What treatments are available?<\/h3><p>There are a plethora of treatments for depression that a doctor can prescribe, from talking therapies to pharmaceutical medications to exercise and more. Sometimes the GP may recommend one treatment or a combination of treatments but they will help support patients and signpost them to services which can bolster their support network.<\/p><p>The good news is that depression is among the most treatable mental health disorders.\u00a0 Between 80% and 90% percent of people with depression eventually respond well to treatment. Almost all patients gain some relief from their symptoms <a href=\"https:\/\/www.psychiatry.org\/patients-families\/depression\/what-is-depression#:~:text=Depression%20is%20among%20the%20most,some%20relief%20from%20their%20symptoms.\">[2]<\/a>.<\/p><h3>Treatment Resistant Depression<\/h3><p>For those with major depressive disorder, however, treatment can be harder to find. With up to one third of adults with the condition failing to respond to treatment, new avenues and treatments are being sought.<\/p><p>\u201cAlthough there is some disagreement as to how to define treatment-resistant depression, a patient is generally considered to have it if the individual hasn\u2019t responded to adequate doses of two different antidepressants taken for a sufficient duration of time, which is usually six weeks,\u201d explains Jaskaran Singh, M.D., Senior Director of Neuroscience, Janssen Pharmaceuticals <a href=\"https:\/\/www.jnj.com\/health-and-wellness\/4-facts-about-treatment-resistant-depression\">[3]<\/a>.<\/p><p>The theory that depression may have causes currently unknown by scientists is one theory researchers are exploring to try and combat resistance to treatment.<\/p><p>\u201cOne of the more modern theories is that depression creates inflammation in the brain, or that inflammation in the brain creates depression. Traditional antidepressants only affect neurotransmitters, so this may be why some patients don\u2019t respond to them.\u201d\u00a0 says Alexander Papp, M.D., a psychiatrist at UC San Diego Health <a href=\"https:\/\/www.jnj.com\/health-and-wellness\/4-facts-about-treatment-resistant-depression\">[4]<\/a>.<\/p><p>Whether or not this is the case, researchers are still trying to find solutions to Treatment Resistant Depression.<\/p><h3>The Potential of Cord Blood Therapy<\/h3><p>One study using umbilical cord blood cells delivered promising results. The study administered concentrated umbilical cord blood cells at a dose of 250 million cells via weekly injections over 4 weeks with 1 week intervals, to women who suffered with Treatment Resistant Depression.\u00a0 The results were tested against a control group who received a placebo.<\/p><p>The study found that the therapeutic potential of umbilical cord blood cell concentrate can be used to overcome treatment resistance formed in depressive patients <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7087609\/\">[5]<\/a>. Other studies have also found that cellular therapies may have a role in combating Treatment Resistant Depression <a href=\"https:\/\/www.mdpi.com\/1999-4923\/15\/3\/814\">[6]<\/a> <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC8474995\/\">[7]<\/a>.<\/p><p>These studies give hope for finding new treatments.
