The therapy aims to strengthen the heart and improve outcomes for children with the condition.<a href=\"https:\/\/cardiovascularbusiness.com\/topics\/clinical\/cardiac-surgery\/first-study-its-kind-focus-new-therapy-hypoplastic-left-heart-syndrome\" target=\"_blank\" rel=\"noopener\">[1]<\/a><\/p>\n<h3 style=\"font-weight: 400;\">What is HLHS?<\/h3>\n<p style=\"font-weight: 400;\">In HLHS, the left side of the heart, which is responsible for pumping oxygenated blood through the body, does not develop properly in the womb. The left ventricle and related structures are small and underdeveloped.<\/p>\n<p style=\"font-weight: 400;\">Before birth, a baby\u2019s circulatory system includes temporary connections between the two sides of the heart: a hole called the foramen ovale and a duct between the pulmonary artery and the aorta called the ductus arteriosus. Normally, these close shortly after birth as the baby begins breathing independently.<\/p>\n<p style=\"font-weight: 400;\">In babies with HLHS, these temporary connections are essential for oxygenated blood to circulate. Once they close, the underdeveloped left side cannot pump blood effectively, and the right side becomes overloaded. Symptoms appear within hours or days of birth and may include a bluish tint to the skin (cyanosis), cold extremities, lethargy, weak pulse, and difficulty breathing. These surgeries are performed in three stages:<a href=\"https:\/\/www.cdc.gov\/heart-defects\/about\/hypoplastic-left-heart-syndrome.html\" target=\"_blank\" rel=\"noopener\">[5]<\/a><a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/hypoplastic-left-heart-syndrome\/diagnosis-treatment\/drc-20350605\" target=\"_blank\" rel=\"noopener\">[6]<\/a><\/p>\n<ul>\n<li style=\"font-weight: 400;\"><strong>Norwood procedure<\/strong> &#8211; Rebuilds the aorta and connects it to the heart\u2019s right ventricle, then links the pulmonary artery to enable the right ventricle to pump blood to the body. Usually performed within the first two weeks of life.<a href=\"https:\/\/kidshealth.org\/en\/parents\/norwood.html\" target=\"_blank\" rel=\"noopener\">[7]<\/a><\/li>\n<li style=\"font-weight: 400;\"><strong>Bidirectional Glenn procedure<\/strong> &#8211; Connects the pulmonary artery directly to the superior vena cava, allowing oxygen-poor blood from the upper body to flow to the lungs, reducing strain on the heart. Typically done between 4\u20136 months of age.<a href=\"https:\/\/kidshealth.org\/en\/parents\/glenn.html\" target=\"_blank\" rel=\"noopener\">[8]<\/a><\/li>\n<li style=\"font-weight: 400;\"><strong>Fontan procedure<\/strong> &#8211; Connects the inferior vena cava to the pulmonary artery, further improving oxygenated blood flow throughout the body. Generally performed at 3\u20134 years of age.<a href=\"https:\/\/kidshealth.org\/en\/parents\/fontan.html\" target=\"_blank\" rel=\"noopener\">[9]<\/a><\/li>\n<\/ul>\n<p style=\"font-weight: 400;\">These procedures allow many children with HLHS to survive into adolescence and adulthood.<a href=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/12214-hypoplastic-left-heart-syndrome-hlhs\" target=\"_blank\" rel=\"noopener\">[4]<\/a> However, the single ventricle remains under strain, eventually leading to heart failure and potential need for a transplant.<a href=\"https:\/\/cardiovascularbusiness.com\/topics\/clinical\/cardiac-surgery\/first-study-its-kind-focus-new-therapy-hypoplastic-left-heart-syndrome\" target=\"_blank\" rel=\"noopener\">[1]<\/a><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10026620\/\" target=\"_blank\" rel=\"noopener\">[10]<\/a> Transplants are limited by donor availability, and traditional heart failure treatments are more technically challenging in HLHS patients due to the unique single-ventricle physiology.<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0146000523000289\" target=\"_blank\" rel=\"noopener\">[11]<\/a><\/p>\n<h3><span style=\"font-weight: 400;\">How could stem cells help?<\/span><\/h3>\n<p style=\"font-weight: 400;\">The stem cell therapy being trialed, laromestrocel, is administered in addition to standard surgery. It consists of mesenchymal stem cells (MSCs) derived from the bone marrow of healthy donors, injected into the heart muscle during the Glenn procedure.<a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10026620\/\" target=\"_blank\" rel=\"noopener\">[10]<\/a> Laromestrocel is an experimental therapy and is not yet approved as a standard treatment for HLHS.<\/p>\n<p style=\"font-weight: 400;\">MSCs have anti-inflammatory and regenerative properties. The therapy aims to strengthen the heart by stimulating new blood vessel growth and supporting the heart\u2019s limited regenerative abilities. This could improve heart performance and may delay or reduce the risk of heart failure and the need for a transplant.<a href=\"https:\/\/cardiovascularbusiness.com\/topics\/clinical\/cardiac-surgery\/first-study-its-kind-focus-new-therapy-hypoplastic-left-heart-syndrome\" target=\"_blank\" rel=\"noopener\">[1]<\/a><\/p>\n<p style=\"font-weight: 400;\">In a previous Phase 1 clinical trial, the therapy was shown to be safe, with no major adverse cardiac events or treatment-related infections.<a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10026620\/\" target=\"_blank\" rel=\"noopener\">[10]<\/a><a href=\"https:\/\/longeveron.com\/clinical-trials\/\" target=\"_blank\" rel=\"noopener\">[12]<\/a> All patients remained alive and transplant-free five years after treatment, exceeding historical survival averages .<a href=\"https:\/\/cardiovascularbusiness.com\/topics\/clinical\/cardiac-surgery\/first-study-its-kind-focus-new-therapy-hypoplastic-left-heart-syndrome\" target=\"_blank\" rel=\"noopener\">[1]<\/a><a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/circ.148.suppl_1.17067\" target=\"_blank\" rel=\"noopener\">[13]<\/a><\/p>\n<p style=\"font-weight: 400;\">The therapy has now advanced to Phase 2b, a randomised, double-blind, placebo-controlled trial. Forty infants with HLHS have been recruited across leading pediatric heart centers in the United States. Twenty will receive laromestrocel during the Glenn procedure, while the remaining 20 will receive standard surgery alone. Initial results are expected within a year, with follow-up planned for up to five years.<a href=\"https:\/\/cardiovascularbusiness.com\/topics\/clinical\/cardiac-surgery\/first-study-its-kind-focus-new-therapy-hypoplastic-left-heart-syndrome\" target=\"_blank\" rel=\"noopener\">[1]<\/a><a href=\"https:\/\/sph.uth.edu\/research\/centers\/ccct\/elpis\/\" target=\"_blank\" rel=\"noopener\">[14]<\/a><\/p>\n<h3><span style=\"font-weight: 400;\">Hope from stem cells<\/span><\/h3>\n<p style=\"font-weight: 400;\">Regenerative medicine has drawn significant attention as a potential approach to strengthen the heart. Researchers are investigating stem cells from various sources, including bone marrow and umbilical cord tissue, to determine the optimal timing and method of administration in HLHS treatment.<a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC12428058\/\" target=\"_blank\" rel=\"noopener\">[15]<\/a><\/p>\n<p style=\"font-weight: 400;\">Laromestrocel is also being explored for other conditions where MSCs\u2019 anti-inflammatory and regenerative properties may be beneficial, such as Alzheimer\u2019s disease and age-related frailty.<a href=\"https:\/\/longeveron.com\/clinical-trials\/\" target=\"_blank\" rel=\"noopener\">[12]<\/a><\/p>\n<p style=\"font-weight: 400;\">For children born today, storing stem cells may offer potential medical benefits in the future. But when feeling down persists for weeks, or even months, it can be a sign of a mental health condition, like depression.<\/p><p>Although today people are well versed and more understanding of depression, some sufferers still find it difficult to find treatments.<\/p><h3>What is depression?<\/h3><p>Depression is a common mental health condition. It is characterised by low mood, feelings of hopelessness, losing interest in things which previously brought a sense of enjoyment, and feeling tearful.<\/p><p>Depression can also manifest in physical ways. Sleeping poorly, feeling tired, experiencing aches, pains, a diminished appetite, and decreased libido can all be impacts of depression. The symptoms range from mild to severe. In mild cases, a persistent low mood may be the cause, while severe cases may result in suicidal feelings or a desire to self harm <a href=\"https:\/\/www.nhs.uk\/mental-health\/conditions\/depression-in-adults\/overview\/\">[1]<\/a>.<\/p><p>Sometimes the causes of depression can be obvious, such as a bereavement or a traumatic event. Other times there might be no known cause, although family history could be the reason. Whatever the reason, once a person has identified they may be depressed, they should go and see their GP.<\/p><h3>What treatments are available?<\/h3><p>There are a plethora of treatments for depression that a doctor can prescribe, from talking therapies to pharmaceutical medications to exercise and more. Sometimes the GP may recommend one treatment or a combination of treatments but they will help support patients and signpost them to services which can bolster their support network.<\/p><p>The good news is that depression is among the most treatable mental health disorders.\u00a0 Between 80% and 90% percent of people with depression eventually respond well to treatment. Almost all patients gain some relief from their symptoms <a href=\"https:\/\/www.psychiatry.org\/patients-families\/depression\/what-is-depression#:~:text=Depression%20is%20among%20the%20most,some%20relief%20from%20their%20symptoms.\">[2]<\/a>.<\/p><h3>Treatment Resistant Depression<\/h3><p>For those with major depressive disorder, however, treatment can be harder to find. With up to one third of adults with the condition failing to respond to treatment, new avenues and treatments are being sought.<\/p><p>\u201cAlthough there is some disagreement as to how to define treatment-resistant depression, a patient is generally considered to have it if the individual hasn\u2019t responded to adequate doses of two different antidepressants taken for a sufficient duration of time, which is usually six weeks,\u201d explains Jaskaran Singh, M.D., Senior Director of Neuroscience, Janssen Pharmaceuticals <a href=\"https:\/\/www.jnj.com\/health-and-wellness\/4-facts-about-treatment-resistant-depression\">[3]<\/a>.<\/p><p>The theory that depression may have causes currently unknown by scientists is one theory researchers are exploring to try and combat resistance to treatment.<\/p><p>\u201cOne of the more modern theories is that depression creates inflammation in the brain, or that inflammation in the brain creates depression. Traditional antidepressants only affect neurotransmitters, so this may be why some patients don\u2019t respond to them.\u201d\u00a0 says Alexander Papp, M.D., a psychiatrist at UC San Diego Health <a href=\"https:\/\/www.jnj.com\/health-and-wellness\/4-facts-about-treatment-resistant-depression\">[4]<\/a>.<\/p><p>Whether or not this is the case, researchers are still trying to find solutions to Treatment Resistant Depression.<\/p><h3>The Potential of Cord Blood Therapy<\/h3><p>One study using umbilical cord blood cells delivered promising results. The study administered concentrated umbilical cord blood cells at a dose of 250 million cells via weekly injections over 4 weeks with 1 week intervals, to women who suffered with Treatment Resistant Depression.\u00a0 The results were tested against a control group who received a placebo.<\/p><p>The study found that the therapeutic potential of umbilical cord blood cell concentrate can be used to overcome treatment resistance formed in depressive patients <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7087609\/\">[5]<\/a>. Other studies have also found that cellular therapies may have a role in combating Treatment Resistant Depression <a href=\"https:\/\/www.mdpi.com\/1999-4923\/15\/3\/814\">[6]<\/a> <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC8474995\/\">[7]<\/a>.<\/p><p>These studies give hope for finding new treatments.
